In order to register your business with FL Construction, you will need to download and complete these forms:

• Vendor Information Sheet

• Subcontractor Agreement

• W9 - Taxpayer Information

• From the W9, please return only the first page

To above documents please add:

• A Certificate of Election of Exemption for each officer listed on the Division of Corporations site, if you do not have Workers’ Compensation coverage

• A copy of your State License if your trade requires State registration

• A Certificate of Insurance, sent from your insurance agent listing the Certificate Holder as:
